I. Background
CD56 是神经细胞粘附分子（NCAM1）的蛋白产物，由 11 号染色体上的 NCAM1 基因编码，是一种具有多个免疫球蛋白超家族结构域和糖基化位点的细胞表面糖蛋白，通过跨膜区域锚定在细胞膜上，并拥有一个胞内尾部，参与细胞内的信号传导。 CD56 在神经系统中广泛表达，包括神经元、胶质细胞等，参与神经元粘附、神经突束化、神经突生长等过程。同时，它也是自然杀伤细胞（NK 细胞）的典型表型标记，根据细胞表面 CD56 的密度，NK 细胞可分为 CD56 Bright 和 CD56 Dim 两类。CD56 Bright 为未成熟 NK 细胞，具有产生丰富细胞因子的能力；CD56 Dim 为成熟 NK 细胞，细胞毒性更强，表达更多免疫球蛋白样受体。在临床应用上，CD56 可作为肿瘤诊断的标志物，特别是在神经内分泌肿瘤和小细胞肺癌中具有较高的阳性率。同时，CD56 的表达也有助于区分多发性骨髓瘤和正常浆细胞，并可作为多发性骨髓瘤治疗的预测性生物标志物。
