| I. Background | |
| TRKB(原肌球蛋白受体激酶 B,亦称 BDNF 受体或 NTRK2)是神经营养因子受体家 族(Trk 受体)的关键成员。作为一种跨膜酪氨酸激酶受体,其特异性结合配体脑源性神 经营养因子(BDNF) 及神经营养因子-4(NT-4),通过激活下游 PI3K/Akt、Ras/MAPK 和 PLCγ信号通路,核心调控神经元存活、突触可塑性、神经递质释放及轴突导向,是维持 学习记忆、情绪平衡与神经发育的核心枢纽。在病理状态下,TRKB 信号功能障碍直接参 与抑郁症、阿尔茨海默病、神经病理性疼痛及癫痫等神经系统疾病,同时作为泛癌靶点, 其基因融合事件可驱动肉瘤、甲状腺癌等实体瘤进展(拉罗替尼/恩曲替尼等 TRK 抑制剂 已获批治疗),而代谢领域则涉及肥胖(下丘脑 TRKB 调控能量平衡)与非酒精性脂肪肝 (肝星状细胞 TRKB 促纤维化),目前针对 TRKB 的靶向策略涵盖小分子激酶抑制剂(抗 癌)、BDNF 模拟肽(神经保护)及基因疗法(递送 BDNF),但需克服血脑屏障穿透、 组织选择性及耐药突变(如 TRKB G667C)等挑战。 | |
